PITTSBURGH (KDKA) – A trolley derailment, which caused significant delays over the weekend appears to have been caused by human error.

Sources told KDKA-TV Investigator Marty Griffin that there is no concern of it being sabotage and was simply operator and supervisor error.

The trolley passed through a red signal and a switch at a low speed, without damaging the switch or the trolley.

The signal violation was reported and the operator was instructed to change ends of the trolley and move back to the platform. When this was done, what had been the lead wheels were now trailing the rest of the trolley and were on a different track, resulting in the derailment.

When the operator passed the signal, they should have gotten out of the trolley and checked to make sure that all the wheels were on the correct tracks.

There should have been a supervisor sent to the scene to check everything out as well. In this case, neither of those things happened and the end result caused significant delays and inconveniences to riders.

According to Port Authority officials, derailments are extremely uncommon, but there are no reported injuries from this incident.
