Job Description

Job Title: Case Manager
Department: Supportive Services
Reports To: Manager of Case Management Services
FLSA Status: Exempt

Purpose: This position is responsible for assisting and supporting clients in of THE PROGRAM to identify, monitor and achieve their goals.

Essential Duties and Responsibilities: include the following. Other duties may be assigned as necessary.

Maintain a caseload as set forth by the Department Manager
Complete appropriate intake procedure within residential center's designated time frame
Work in conjunction with Department Manager to complete client orientation within center's designated time frame
Work with client to complete and maintain service plan
Administer handbook tests to clients within designated time frame
Create and maintain a file for all clients served including documentation of a minimum of one formal weekly contact per client on caseload
Track, document, and maintain all pertinent medical and legal information on assigned clients
Complete and prepare resident visitor packets and submit to Center Management
Ensure that clients follow proper rules, regulations and procedures in accordance with agency handbook
Perform monitor duties as needed including but not limited to drug screenings, room searches, personal searches, etc.
Complete resident's pharmaceutical documentation and maintain communication with pharmacy regarding resident prescription needs.
Review and approve all client community passes, weekly schedules and budgets
Educate clients on how to access community resources independently
Facilitate team meetings for client review and adjustment hearings
Advocate for clients at all necessary court hearings
Prepare and submit necessary parole papers or other legal documentation as required
Maintain and document weekly communication with therapists and/or additional agency staff regarding clients on caseload.
Attend and participate in all necessary meetings
Prepare monthly and annual reports on client statistics with supporting documentation for agency funding sources.
Initiate, document and maintain a minimum of one new contact per month with community providers and/or area employers.
Participate in professional development training including but not limited to working with offenders and substance abuse related topics.
Be available to work flexible schedule and perform monthly on-call duties as set forth by Center Manager
Promote the mission and goals of the agency at all times

Job Title: Cook
Department: Residential Centers (TPC and ACTA)

Purpose: This position is responsible for the ordering and preparation of food for the facility. This position is also responsible for the cleanliness and maintenance of the kitchen area and equipment.

Essential Duties and Responsibilities: include the following. Other duties may be assigned as necessary.

Prepare all lunch and dinner meals for the residents during the week
Train residents to assist in food preparation and to serve meals in his/her absence
Place food orders and make necessary purchases while staying within the designated budget
Accept delivery of provisions and stores them appropriately
Plans breakfast and weekend meals and leaves adequate instructions for provision of these meals by the residents
Prepares and posts weekly food menus
Completes regularly scheduled inventories of food and cleaning items
Conducts an orientation with all new residents regarding building detail responsibilities
Assigns resident details, if necessary
Monitors and records food temperatures to meet health and safety standards
Has knowledge and follows all necessary Department of Health regulations regarding food preparation and distribution
Research and develop new relationships and maintain existing relationships with food service vendors
Participate in formal evaluation of resident progress
Communicate with on-call staff any unusual incidents, and document all unusual incidents
Report resident violations

Purpose: This position is responsible for the security of residents and the facility. This person works as part of the Corrections Team.

Essential Duties and Responsibilities: include the following. Other duties may be assigned as necessary.

Be available to a work flexible schedule as set forth by the Center Management
Have current knowledge of the criminal justice system and human services systems
Ensure that clients follow proper rules, regulations and procedures in accordance with Resident Handbook
Responsible for reading daily log
Responsible for reviewing and responding to e-mail upon arrival and before departure from scheduled shift
Responsible for sending accurate and timely e-mails regarding paroles, revocations and escapes as well as other vital communications
Perform and document a minimum of three physical head counts per eight-hour shift
Perform and document hourly security checks throughout facility
Verify and record all medications brought into the facility
Administer on-site drug and alcohol tests residents as required
Conduct pat-down/personal searches of residents' and their belongings as required
Conduct searches of residents' rooms, common areas and building perimeter as required
Conduct daily employment verifications per shift per shift with current employers through either telephone consultation or on-site visits
Perform front office duties including entry-exit procedures, answering phones and daily log documentation
Verify identification and register visitors and vendors
Escort residents on leisure and volunteer activities in the community as required
Observe and document medication distribution at designated times
Distribute and document the distribution of resident funds
Oversee weekend meal preparation by the residents
Visually verify and document completion of resident detail
Participate in formal evaluation of resident progress

Purpose: This position is responsible for client drug and alcohol assessment and providing education and counseling to individuals and groups surrounding substance abuse and mental health issues.

Essential Duties and Responsibilities: include the following. Other duties may be assigned as necessary.

Maintain a caseload of residential clients as set forth by the Clinical Supervisor.
Have current knowledge of Pennsylvania Department of Health and other applicable state regulations
Maintain standards of compliance for Pennsylvania Department of Health licensure
Have current knowledge of the criminal justice and human services systems
Complete initial client screening and intake to determine the need for drug and alcohol treatment
Work in conjunction with Case Managers, Corrections and Supportive Services staff
Plan and facilitate educational lectures and support groups
Keep and maintain detailed records on clients' attendance and progress
Maintain appropriate, accurate, pertinent and confidential documentation for client file
Ensure that clients follow proper rules, regulations and procedures in accordance with agency handbook
Maintain and document a minimum of one weekly contact regarding client progress
Prepare monthly reports on client statistics
Attend and participate in all necessary meetings
